Dear N[s. Johnson,
Your Conditional Use Permit application to allow modification and continued operation of an
existing; Sprint wireless telecommunication facility located at 700 W. Hamilton Avenue has
remained incomplete since January 20, 2012. Pursuant to Section 21.38.040(C) of the Campbell
Municipal Code, an application may remain incomplete for six months, after which time the
applicant may request an additional six month extension with cause or the application shall be
deemed withdrawn. On June 20, 2012, the City automatically extended your application the
additional six months as a courtesy.
Therefore, this application will expire on January 20, 2013 if a revised submittal addressing all
completeness comments is not received by this date. Alternatively, you may submit a written
request to withdraw the application and receive a refund of unused fees. If the application is
allowed to expire without a written request for a withdrawal, all fees shall be forfeit and a new
application, including new fees, will be required to recommence processing of this project.
Regardless of whether the application is allowed to expire or formally withdrawn, all equipment
associated with this project shall be removed by no later than July 20, 2013. If you should have
