Bascom Ave, Campbell 95008 Inspector Also Present (Name) Ryan Fox Inspection Report Comments: West Valley Closed Landfill Quarterly Inspection September 2019 Inspection Date/Time: 9/5/19 from 3 pm- 5pm General Notes: Revere Campbell Apartments (Land and Houses U.S.A INC): 1725 S. Bascom Ave. (APN# 288-02-036) Notes: 1. At time of inspection gas probe GD2-10 had a high reading of 11.6% within Revere Campbell’s gas monitoring system. This high percentage triggered the warning alarm to go off and has happened multiple times per Community Director, Jasmine Medina. The facility contacted the gas monitoring system maintenance provider to look at the probe MDC but had not received a response. Inspector located the gas probe GD2-10 to determine the issue. Location of the probe was in the pool/spa pump room behind commercial space for Orange Theory fitness. Pump room access is through the parking lot. Pump room holds all the chemicals and pumping equipment for the facility and appeared to have one pump that had recently overflowed, and chemicals acid/bases appeared to be improperly stored (Sodium bicarbonate on top of bleach). Inspector notified facility operator of findings. Facility contacted pool company following inspection. LEA inspector will follow up for a resolution. 2. “Big Mug Coffee Roaster” has started its new lease in Revere Campbell’s last remaining open commercial unit along Bascom Ave August 2019. Coffee shop likely to install additional plumbing below the foundation to conduct business operations. Discussed that any breaking of foundation that will damage the methane mitigation barrier will require a Post Closure Land Use application submittal and LEA approval prior to any construction activities. Management group to inform inspector of changes as the project moves forward. 4 out of the 5 of Revere Campbell’s commercial spaces along Bascom Ave are filled at time of inspection. a. Suites include: i. 1000 - IVO Salon ii. 1022 - Land and Houses (new) iii. 1050 - Bay West Development iv. OPEN - ***Proposed “Big Mug Coffee Roaster” no contact with LEA as of inspection date. Discussed with ownership group on requirements. v. 1150 - Orange Theory 3. Control room binder is being updated by staff and MDC with every special occurrence. Real time display of the gas monitoring system is located adjacent to the property manager’s office. Records of maintenance and calibration are stored in binder near the display monitor. In accordance to the Operation and Maintenance Manual for the Methane Closed Disposal Site Inspection Report (188) No Violations or Areas of ConcernX Enforcement Agency: Local Inspection ID: County of Santa Clara THE ABOVE FACILITY WAS INSPECTED FOR COMPLIANCE WITH APPLICABLE SECTIONS OF DIVISION 30 OF PUBLIC RESOURCES CODE (PRC) AND TITLE 14 AND TITLE 27 CALIFORNIA CODE OF REGULATIONS (CCR) 1 of 3CalRecycle Solid Waste Information System (SWIS) Mitigation System dated August 5, 2015, all routine maintenance, supervisory alerts and tasks associated with the system must be recorded in a logbook and an annual summary report prepared by the responsible party. a. Recent Service Events: i. 7/29/19 Gas sensor Alarm trouble shooting + calibration ii. 8/22/19 GL55 Sensor Alarm troubleshooting + calibration 4. New gates installed at facility entryways. Methane mitigation barrier maintained. Portfolio Realty Management: 901 Campisi Way (APN# 288-03-017 & #288-02-034): Notes: 1. Observed that the gas monitoring system for building 901 Campisi Way was functioning properly. System was calibrated by Sierra Monitoring Cooperation on 5/17/19. Next calibration set to take place 11/17/19. All monitors operational with 0 readings. Facility needs to maintain requirements and keep system up to manufacture’s specifications. Continue to perform system calibration and submit results to the LEA. 2. Sidewalk walkway closure signs are posted on the 901 Campisi Way side of Creekside way by the city of Campbell. Signs are to block non-ADA compliant walkway. A flashing crosswalk has been added to allow pedestrians to safely cross the street. 3. Parking lot continues to experience differential settlement. Multiple fissures in the parking lot have been repaired since previous quarterly inspection. The property management personnel are generally proactive in making timely repairs to locations found to have open voids along the building and in the strip landscaping and cracks in the parking lot pavement. Campisi Way Associates, LLC: 910 Campisi Way (APN# 288-03-021): Notes: 1. Gas monitoring system was observed to be fully functioning at time of inspection. All monitors were operational with 0 readings. 2. Differential settlement is still occurring around the building of this property. Continue to be proactive in making timely repairs to fill and repair all open voids discovered around the property, especially along the edges of the building. City of Campbell (Campisi Way and Creekside Way) Note: 1. Three sidewalk closure signs along with a flashing pedestrian crosswalk are currently posted along Campisi way. Flashing pedestrian crosswalk sign is to enable pedestrians to safely cross streets and avoid sidewalk closures. 2. Differential settlement has occurred along Campisi Way exposing the ground underneath the road and the metal brackets along the public sidewalks. Pea gravel fill material has also been brought in to fill gaps between the sidewalk and the road. The sidewalk has settled by more than two feet since the pier constructed roadway was constructed in 1989 (almost 30 years ago) and may be no longer in compliance with the Americans with Disabilities Act (ADA) because of excessive slopes. A plan has been approved by the City of Campbell to repair the damaged sidewalks and roadway along Campisi way [Campisi Way Feasibility Study (3/3/2018)]. 3. If construction occurs that requires excavation below the ground surface, the city is required to contact the LEA to ensure public health and safety is maintained. San Jose Water Works: (APN# 288-03-003): Note: 1. No issues observed. The Garret: 1777 S. Bascom Ave. (APN# 288-02-030 & #288-03-018): Note: 1. No signs of differential settlement have been observed. Pruneridge South Bascom/Northview Real Estate Investors LLC: 1845 S. Bascom Ave. (APN# 288-03-019) Note: 1. This notification shall include the name(s), address(es) and phone number(s) of the new owner or operator.
