Dear Mr. Suiter:
The Planning Division has reviewed your business description received November 18, 2004, for
the establishment of your fitness business in an existing shopping center at 150 N. San Tomas
Aquino Rd. The Planning Commission on December 8, 1998, previously approved an indoor
fitness facility at this location for Hartke World of Baseball per Resolution No. 3212., a copy of
the adopted resolution is attached. Based upon your business description, the Community
Development Director has found that your proposed fitness use is similar to that use previously
approved by the Planning Commission and consequently a new use permit or a modification to
the previous use permit is not warranted.
This determination is based upon you statement that a maximum number of persons at the
facility will not exceed 75 persons at anyone time and that the facility is a fitness training
facility that will not accommodate large group or aerobic classes. In the event that the nature of
the business changes, you may be required to process a modification to the prior Conditional Use
Permit.
This determination is subject to the following conditions:

1. Approved Project: Approval is granted for a Zoning Clearance for the establishment of a
14,130 square foot fitness facility in an existing commercial building located at 150 N. San
Tomas Aquino Rd.. Previous Conditional Use Permit approval granted per Planning
Commission Resolution No. 3212 .

2. Operational Hours: The operational hours shall be limited to 7:00 a.m. to 9:00 p.m. Monday
through Friday and 8:00 a.m. to 7:00 p.m. Weekends.
3. Revocation of Permit: Operation of the use in violation of the prior Conditional use Permit or
any standards, codes, or ordinances of the City of Campbell shall be grounds for
consideration of revocation of the Administrative Planned Development Permit by the
Community Development Director.
4. Parking, Driveways, and Maintenance: All parking and driveway areas shall be maintained
in compliance with the approved plans and Chapter 21.28 (Parking and Loading) of the
Campbell Municipal Code. All parking areas shall be regularly swept and cleaned to remove
litter and debris from the parking areas and driveways.
5. Outdoor Storage: No outdoor storage is permitted on the subject property. No equipment,
materials or business vehicles shall be parked and/or stored outside the building or within the
parking lot.
6. Signage: No signage is approved as part of the development applications approved
herein. No sign shall be installed until a Sign Permit is approved as specified in the Sign
Ordinance.
Building Division:
7. Permits Required: A building permit application shall be required for the proposed tenant
improvements to the existing commercial structure. The building permit shall include
Electrical/Plumbing/Mechanical fees when such work is part of the permit.
8. Construction Plans: The conditions of approval shall be stated in full on the cover sheet of
construction plans submitted for building permit.
9. Size of Plans: The size of construction plans submitted for building permits shall be 24
inches by 36 inches.
10. Plan Preparation: This project requires plans prepared under the direction and oversight of a
California licensed Engineer or Architect. Plans submitted for building permits shall be "wet
stamped" and signed by the qualifying professional person.
11. Site Plan: Application for building permit shall include a competent site plan that identifies
property and proposed structures with dimensions and elevations as appropriate. Site plan
shall also include site drainage details. Site address and parcel numbers shall also be clearly
called out. Site parking and path of travel to public sidewalks shall be detailed.
12. Title 24 Energy Compliance: California Title 24 Energy Standards Compliance forms shall
be blue-lined on the construction plans. Compliance with the Standards shall be
demonstrated for conditioning of the building envelope and lighting of the building.
13. Special Inspections: When a special inspection is required by U.B.C. Section 1701, the
architect or engineer of record shall prepare an inspection program that shall be submitted to
the Building Official for approval prior to issuance of the building permits, in accordance
with U.B.C. Section 106.3.5. Please obtain City of Campbell, Special Inspection forms from
the Building Inspection Division Counter.

14. Title 24 Accessibilitv - Commercial: On site general path of travel shall comply with the
latest California Title 24 Accessibility Standards. Work shall include but not be limited to
accessibility to building entrances from parking facilities and sidewalks.
15. Title 24 Accessibility - Commercial: Projects seeking to use the Title 24 Hardship
exemption clause shall blue-line completed, City of Campbell "20%" exemption form on
submitted construction plans. Form is available at Building Division service counter.
16. Other Agency Approvals: The project requires the following agency approval prior to issuance of
the building permit:
a. West Valley Sanitation District
b. Santa Clara County Fire Department
18. P.G.& E.: Applicant is advised to contact Pacific Gas and Electric Company as early as
possible in the approval process. Service installations, changes and/or relocations may require
substantial scheduling time and can cause significant delays in the approval process. Applicant
should also consult with P.G. and E. concerning utility easements, distribution pole locations
and required conductor clearance.

Written Statement For Perennial Fitness. Inc. Location At San Tomas Plaza
Proposal: To utilize the Hardtke Baseball facility as a neighborhood type fitness facility,
providing similar services. There would be no construction or alteration of the structure, there
would be extensive upgrading of aesthetics performed, such as painting and carpeting.
Type of business:
A neighborhood fitness facility providing individuals (male and female) the opportunity to
maintain a fit lifestyle, bettering their health and quality of living. The facility offers circuit type
training as well as personal training and sport specific training, through Certified Personal
Trainers. There would be a membership base as well as per session, walk-in use offered. This
business will give opportunity to a base of customers that might not otherwise be able to afford
or be inspired to join a fitness facility, as there will not be lengthy contracts, committements or
initiation fees.
Size of facility:
This will be a 14,130 square foot facility, with 6,500 square feet devoted to membership and
walk-in use, 4,000 square feet devoted to personal training, and approximately 750 square feet
retail.
Traffic generation:
Traffic generation would be minimal, there would not be large "aerobics" or "group" class
scenarios, a mass influx of individuals at one time would not be expected. Estimated maximum
number of simultaneous arrivals at peak periods would be 20 - 25 persons. Maximum
estimated number of persons using the facility at one time would be approximately 75 (64
members or walk-ins, and 11 personal training clients).
Parking:
It is not anticipated that a large number of people would arrive at one time, so parking should
not be influenced dramatically. It would be requested of members that they use the side parking
area of the complex, so as not to impact the parking for surrounding businesses. This is a
fitness facility, and most people are here to exercise, so they don't tend to mind the additional
footsteps. Those who choose to park in the area in front of the facility, would thus be
minimized.
Employment:
It is estimated there will be 2 - 6 employees on staff at any given time of operation.
As well as providing scheduled employment to non-skilled persons, this business will offer
Certified Personal Trainers the opportunity to generate income where other fitness facilities fail
to. Most facilities require trainers to work only with members of that facility. Many trainers know
individuals who would like to train with them but are not allowed to because either the trainer or
the client does not "belong" to the same facility. Also, most facilities require a trainer work a
minimum amount of hours and shift times, eliminating those who can only work limited hours
due to family obligations, etc. Trainers using this facility will have the opportunity to train clients
whether they are members or not, and work a schedule set entirely by the trainer.
Added benefit:
This business would also bring people into the shopping center that might not otherwise
frequent it, bringing business to the surrounding merchants. It would especially benefit the
Long's Drugs, The Dollar Store and the Coffee Shop, people that would be inclined to visit our
facility would also be the type to utilize those services, as well as others in the Plaza.
Proposed hours of operation:
7:00 a.m. until 9:00 p.m., Monday - Friday.
Saturday 8:00 a.m. until 7:00 p.m.
Sunday - Closed.
I have been a resident of Campbell since 1991, and in the fitness industry since 1995. I have
always generated my income from locations on the outskirts of Campbell, and am looking
forward to bringing revenue into the city of Campbell, to help better the community in which I
reside. I will constantly and diligently pursue the highest standards at this facility.
Hopefully, this is the start of a growing company that will be based in Campbell.
