I4 Own r Contractor Architect/Designer __Agent Rppli ant Signature Printed Name Date MEMORANDUM CITY OF CAMPBELL TO: Bill Bruckart, Building Official DATE: 03/06/20 FROM: Arlyn Villanueva /Roger Storz, Engineer(408) 866-2153 SUBJECT: BUILDING PERMIT APPLICATION Site Address: 137 Llewellyn Avenue For File No(s): BLD-2020-291 Project Description: Building permit to build an addition and remodel of an existing dwelling Applicant: Michael Vu COMMENTS The scope of this project triggers the requirement for Frontage Improvements as required by Campbell Municipal Code 11.24.040. This is an older neighborhood constructed with attached sidewalk. The existing driveway does not meet accessibility (ADA) requirements and must be reconstructed as part of this project. As this is a relatively minor frontage improvement, the applicant can apply for an Encroachment Permit and hire a qualified concrete contractor to install a new accessible driveway curb cut per the attached City Standard Detail No. D-15. The applicant shall apply for and be issued the encroachment permit prior to issuance of the Building permit, and the new driveway shall be constructed before the applicant can obtain occupancy. PUBLIC WORKS DEPARTMENT CONDITIONS OF APPROVAL 1. Lot Lines: Verify the dimensions of the property lines. The property lines distances do not match the property lines distances on Tract No. 1283 and the APN map, looks like the lot layout is in reverse. 2. Water Meter: The project has an existing water meter installed in the public right-of-way. If the water service is required to be upsized as part of the project(i.e. due to fire sprinklers),then the new water meter shall be installed on private property behind the public right-of-way line. 3. The following conditions only apply if the applicant has a need to install / upgrade utility services (water, sewer, gas, etc.) in the street: a. Utility Encroachment Permit: Separate permits for the installation of utilities to serve the development will be required(including water,sewer,gas,electric,etc.). Applicant shall apply for and pay all necessary fees for utility permits for sanitary sewer, gas, water, electric and all other utility work. b. Utility Coordination Plan: Prior to issuance of building permits for the site, the applicant shall submit a utility coordination plan and schedule for approval by the City Engineer for installation and/or abandonment of all utilities.The plan shall clearly show the location and size of all existing utilities and the associated main lines; indicate which utilities and services are to remain; which utilities and services are to be J:\LandDev\Conditions\Bldg\Llewellyn Avenue 137 03-06-2020 Page 1 of 2 • 137 Llewellyn Avenue (BLD-2020-291) abandoned, and where new utilities and services will be installed. Joint trenches for new utilities shall be used whenever possible. c. Pavement Restoration: Based on the utility coordination plan, the applicant shall prepare a pavement restoration plan for approval by the City Engineer prior to any utility installation or abandonment. Streets that have been reconstructed or overlaid within the previous five years will require boring and jacking for all new utility installations. Llewellyn Avenue has not been reconstructed or overlaid in the last 5 years. The pavement restoration plan shall indicate how the street pavement shall be restored following the installation or abandonment of all utilities necessary for the project. 4. Encroachment Permit/Fees/Deposits: The applicant shall obtain an encroachment permit (including fees, surety and insurance) for construction of the following standard public street improvements (Revise plan to show this work): a. Construction of a new ADA compliant driveway approach. b. Show the existing and new public improvements along project frontage. c. Show location of all existing utilities within the existing public right of way. And new location for existing utilities that are being relocated. d. Add a note on plan that states: CONTRACTOR WILL BE REQUIRED TO OBTAIN AN ENCROACHMENT PERMIT PRIOR TO ANY WORK IN CITY OF CAMPBELL RIGHT-OF-WAY. 5. Street Improvements Completed for Occupancy and Building Permit Final: Prior to allowing occupancy and/or final building permit signoff for any and/or all buildings,the applicant shall have the required street improvements and any pavement restoration installed and accepted by the City. 6. Maintenance of Landscaping: Owner(s), current and future, are required to maintain the landscaped park strip and tree wells in the public right of way.
