I am an interested party in the potential purchase and leasing of a property in Campbell. Endosed
please find our Bus]ness Licensh Application.
The property is 1570 La Pradera Avenue, currently used as a Beauty Salon.
The property is zoned as C-58.
I will be the Director and sole full time employee of the Donnadae Foundation ("DDF').
We will be using 1570 La Pradem as the worldwide headquarters of the DDF. We will use the
building as executive offices for administrative and management purposes. In addition, we will keel~
the building as a beauty salon with pa_st
objective at our foundation offices/nd incidental to our main use, we will conduc~ group sessions
and/or instructional meetings, v _
I believe that our use is befitting the C-58 zoning.
Operational hours would be 7:00 am to 9:00 pm daily.
Our use would require no structural changes to the exterior of the building and minimal char~es to a
few of the internal'walls if we were to pur~ase the above referenced property (i.e. the dosing hfone
doorway and the opening of another).
I do not have access to the formal site plan for this property;, however, please see attached metro scan
of property. The building is approximately 2000 sq. fi, single story, semi-rectangular building.

made to Hospice, a non-profit organization that cares for those in their last 6-8
months of life. This money is used to give an everyday pleasure to a patient that
makes their heart feel most alive like flowers, ice cream, music or movie tickets. At
the same time, the intention of each piece is to bring out a personal feeling in you
unveiling a perspective that inspires you to seek more in the quality of your life.
