to subject "s" approval for the construction of a medical/dental
buildir..g to be located at 1580 l<iinchester Boulevard in the
Ci~y of Campbell are as follows:
1. Conformance with Section 9126 of ~le Campbell Municipal
Code to include the following~
a. Dedication to plan line right of way on Hamilton
i\venue (120') and Winchester Boulevard (90').
b. Full street improvements on Winchester Boulevard.
c. Stor.m drain fee in the amount of $640.00.
When all of the above requirements have been complied with,
this office will recommend the issuance of a building perIuit.
Compliance shall be accomplished by:
1. Execution of Deed which shall be drawn by the City of ~
Campbell upon notification of intent to proceed and the correct
vesting_
2. Submitting acceptable street tmprov~nt plans to the
City Engineer and obtaining- hJ.s approval of eaine.
3. Posting of Faithful Performance Bond in the amount of lOO%
of. the stimated coat of the ,street imp:' ovements. \
4. Submit'c.tng check in the amount (:)f '.!S40.00 for storm ~

No building permit shall be is~ued after the expiration date of any
approval until a new approval has been obtained in the mannor provided
for in this chapter.
