Dear Mr. Dyer:
The Planning Division has reviewed and conditionally approved your sign application
(PLN2003-71) for one new wall sign at 1600 W. Campbell Avenue. The new wall sign is
32.7 square feet and consists of halo-illaminated, individual letters reading "Campbell
Family Practice".
This Sign Permit has been approved subject to the following conditions of approval:
1. The applicant shall obtain any necessary building permits from the Building
Division prior to sign installation.
2. The proposed sign shall be fabricated and installed per the approved plans dated as
approved on May 29, 2003.
3. The sign shall be maintained in good condition at all times and shall be repaired or
replaced as necessary.
