Dear Mr. Thomas:
The Planning Division has reviewed your revised building permit plans for a new stealth wireless
(Monopine) telecommunications facility located at the above referenced address. The following
issues and/or conditions of approval need to be addressed prior to the issuance of a building
permit:
1. Sheet A-OS and A-06: The revised plans do not substantially conform to the approved plans
dated February 24, 2005. The tree's form is inconsistent with the original plans. I have
enclosed a copy of the approved elevations for your review.
2. Maintenance of Simulated Material: The letter submitted was not signed by the author
have enclosed the original letter for Mr. Moscola's signature.
Please submit your revised plans to the Building Division. Only sheets that have been modified
need to be submitted with your revision. If you have any questions please do not hesitate to
contact me at (408) 866-2140 or by email at melindad(a~cityofcampbell.com.

Re: CAMPBELL SF0196 site
Dear Mr. Gupta:
The Monopine monopole should be inspected annually for the following:
1. The branches if found broken and/or loose should be replaced and/or
tightened as applicable.
2. The antennas and support connections if found loose or damaged should
be tightened or replaced as applicable.
3. Check the color of the monopole, branches and antenna canisters for
significant fading and/or chipping. The damaged components should be
repainted.
All above maintenance requirements are to be performed by the owner of the
Monopine.
Sincerely,
Joe Moscola
