The Administrative Site and Architectural Review Permit application to allow the modification
and continued operation of an existing roof mounted AT&T Wireless Telecommunication
Facility located at the above referenced address has remained incomplete since July 30, 2010. No
formal response to the incompleteness letter has been received. Pursuant to Section 21.38.040(C)
of the Campbell Municipal Code, an application may remain incomplete for up to 180 days, after
which time the application shall be deemed withdrawn with no refund of fees paid and no action
taken.
Therefore, the Administrative Site and Architectural Review Permit application has expired and
is closed with no action taken.
If you are still interested in pursuing an Administrative Site and Architectural Review Permit for
your proposed project, you will be required to submit a new application for review. This will
include new submittal documents and the payment of all fees. If you should have any questions

for to allow the continuation of an existing AT&T Wireless Telecommunication Facility and the
installation of new roof mounted antennas and equipment. Your application has been deemed
incomplete. The following additional information and details are needed in order to deem your
application complete:
1. Pro~ertv Owners Signature: Any application for City approval of a project must include
the property owner`s information and siga~ature in order to be valid.
2. Plan Sets: Provide four complete copies of the submitted plans in an 11 inch by 17 inch
format.
