SPECIAL PROVISIONS:
1. Street shall not be open cut for underground installations. Minimum cuts may be allowed for connections or exploration holes. Such cuts maybe
~ecificall~aparoved by the Inspector prior to cutting.
~2. Pavement may be cut for underground installations and must be restored in accordance with the Standard Details Trench Restoration Method
"A", unless otherwise approved by the Engineer.
3. Work to be staked by a licensed Land Surveyor or Civil Engineer and two (2) copies of the cut sheets sent to the Public Works Department

4. Per Section 4215 of the Government Code this permit is not valid for excavations until Underground Service Alert (USA) has been notified and

GENERAL PERMIT CONDITIONS
1. A Construction Cash Deposit is required. Charges will be made against this deposit if there is an emergency call-out, overtime inspection or when City
ordered barricading is required. Any such costs in excess of the deposit will be billed to the Permittee.
Z. A one-veer maintenance period and surety are required. Such period will begin on date of written acceptance by the City.
3. Refund of the cash deposit balance and refund or cancellation of the Faithful Performance Surety will be initiated by the written acceptance of the work
by the City.
4. The Permittee must request in writing a final inspection and acceptance of the work upon completion. Acceptance by the City will be made in writing [o
the Permittee.
5. Maintain safe pedestrian and vehicular crossings and free access to private driveways, bus stops, fire hydrants and water valves.

6. A Construction Traffic Control Plan and a Construction Schedule are required for all lane closures, detours and street Closures. i his plan must be
reviewed and approved prior to any lane closures.
7. The Construction Traffic Control Plan shall conform to the Caltrans Manual of Traffic Controls for Construction and Maintenance Work Zones, dated
1990, available at Caltrans. Traffic control equipment shall include Type II flashing arrow signs if required.
8. Replace as directed by the City Engineer any damaged or removed improvements in accordance with City Standards and Specifications at the sole
expense of the Permittee.
9. Sawcut for all PCC or AC removals. All PCC removals shall be to nearest scoremark and shall be doweled to existing improvements.
10. Prior approval of inspector is required for any work done after normal working hours, on weekends or holidays and may require reimbursement of
inspection costs at the current overtime rate.
11. Adequate signing and bamcading is required on the job site. Failure to provide such signing and barricading may result in the City's providing signing
and bamcades and charging the cost (including all labor and materials) against the cash deposit.
12. Compaction testing of subgrade, base rock, and asphalt concrete by Permittee is required unless otherwise stated by the City Engineer.
13. The Contractor or Permittee will have a supervisory representative available for contact on the project at all times during construction. Contntctor or
Permittee shall provide a phone number at which they can be contacted outride the hours of 8:00 a.m. to 4:00 p.m.
14. No storage of materials or equipment will he allowed near the edge of pavement, the trave{ed way, or within the shoulderline which would create a
hazardous condition to the public.
15. This permit shall not be construed as authorization for excavation and grading on private property adjacent to the work or any other work for which a
separate permit may be required, nor does it relieve the Permittee of any obligation to obtain any other permit required by law.
1 G. This permit does not release the Permittee from any liabilities contained in other agreements or contracts with the City and any other public agency.
17. This permit is not transferable. Work must be performed by the Permittee or his designated agent or contractor as specified thereon.
18. Ca11 back (call out) due to emergencies regarding this permit shall be at the current overtime rate with a three (3) hour minimum charge per occurrence.
19. Pursuant to Chapter 14.02 of the Campbell Municipal Code, applicant shall not cause to he discharged any material into the municipal storm drain system
other than storm water. Applicant shall adhere tb the BEST MANAGEMENT PRACTICES established by the Santa Clara Valley Urban Runoff
Pollution Prevention Program.
20. (f the public interest requires a modification of, or a departure from, the plans and specifications, the City shall have the authority to require or approve
any modification or departure and to specify the manner in which the same is to be made for City-owned or maintained facilities.
21. Permittee must provide advance notification to all parties that may be affected by the permit activities. Notification shall be reviewed by City prior to
distribution and include dates of work and a contact name and phonc number.

AT&T PROJECT LIGIiTSPEED -SPECIAL PROVISIONS
1. AT&T shall provide the City of Campbell a current map indicating all SAI cabinet locations.
2. All proposed facilities shall be of consistent materials, finish and color (except where otherwise agreed, existing cabinets will be painted to
match paired Lightspeed cabinet). Pedestal numbering shall be consistent and as inconspicuous as possible. New Lightspeed cabinets shall have
an anti-graffiti finish.
3. Plans shall be accurately dimensioned, clearly showing the proposed location, size, and shape of all existing and proposed facilities, including
all existing and proposed right-of--way lines and any existing or proposed easements.
4. Applicant shall submit photos of the proposed location both pre-construction and post construction (with proposed structure(s) and all related
service pedestals, bollards, landscaping, screening, etc. shown).
5. • Plans shallreflect that the following guidelines whenever possible. The proposed facilities should be:
a. located outside of any future sidewalk area
b. located outside and beyond the back of walk (existing or future) wherever possible, with a minimum clearance of 18" from the
sidewalk
c. in park strip locations (only considered if beyond the back of walk location is not feasible), located with a minimum clearance of
18" from the face of curb to any part of the facility (if bollards are proposed, bollards must be located a minimum of 18" from
face of curb), and a minimum of 18" from existing sidewalk
d. in sidewalk locations (only considered if beyond the back of walk locations are not feasible), located to accommodate the
required ADA specifications, with no less than a 4' clear path around the proposed facilities and with a minimum clearance of
- 18" from the face of curb to any part of the proposed facility (including bollards)
e. located 5' from any existing public improvements such as, trees (tree wells if applicable), light poles, sign poles, etc.
f. located with adequate clearance from existing fire hydrants
g. located with adequate line of sight clearances
6. All contractors working under the permit shall have a valid City business license.
7. Applicant shall obtain approval from other agencies (Caltrans, Santa Clara Valley Water District, VTA, etc.) as needed.
8. After the City has reviewed and conditionally approved each permit location, the applicant shall proceed with the public/property owner
notification process as follows:
a. Following the City's preliminary approval of a site, AT&T shall mail a preliminary notice to all property owners that are adjacent
to, or abut, the proposed location, as well as to those across the street from the location. In some cases, the City may request
notifications be sent to additional properties or parties. The preliminary notice, which shall be approved by the City prior to
mailing, shall be mailed to the property owners, with a copy to the City, not less than four (4) weeks prior to anticipated
construction date, and shall give the property owner a two (2) week period in which to respond with questions or concerns.
Should the notified property owner have concerns regarding the location of the facilities, AT&T shall work with the property
owner(s) toward a mutually acceptable location.
b. AT&T shall provide a final notice to the property owners at least 48 hours before the commencement of the proposed
construction.
9. Applicant shall ensure that all property owner notifications have been completed prior to construction.
10. AT&T agrees to maintain the facilities in an attractive and clean manner. AT&T will make commercially reasonable efforts to remove graffiti
within 48 hours of notification that the facilities have been tagged. AT&T shall provide the City with contact information for graffiti removal.
11. The City of Campbell will bill AT&T if its contractor leaves any site in such a manner requiring City forces to mobilize and mediate an unsafe
situation.
12. If and only to the extent required by State or federal law, should a public project arise that requires the relocation of existing facilities, the
facilities shall be removed/relocated to accommodate the scope, alignment and schedule of the public project, in accordance with state or federal
legal or regulatory requirements.
13. As required by State statues, and unless otherwise preempted by State or Federal Law, abandoned facilities shall be completely removed from
the site and the site restored to its original condition.
14. The parties agree to fully reserve their rights as to the applicability and interpretations of California Public Utilities Code Section 7901 and other
related local, state or federal laws, and the City reserves the right to contend that future permits will not be issued without AT&T obtaining a
franchise or other agreement prior to providing video services, and AT&T reserves it's right to contend that City is required to issue the
encroachment permits without first obtaining such franchise or agreement.

The City of Campbell has completed the preliminary review of the subject encroachment permit
application. In order to complete the review process of this permit application, it is imperative
that the following comments are addressed.
1. Submit updated map of all existing SAI cabinets within the city boundary.
2. Submit revised Campbell Resident Notification letter with appropriate photo exhibit(s).
3. Submit signed letter to the Director of Public Works regarding AT&T's compliance with
provisions of AB 2987.
4. Review and ascertain compliance with all provisions as stated in the enclosed AT&T
Project Lightspeed -Special Provisions.
5. Sign and return the enclosed AT&T Project Lightspeed -Special Provisions.
6. Placement of proposed bollards shall be in compliance with Special Provisions. Revise
drawings for encroachment application.

GENERAL PERMIT CONDITIONS
I. A Construction Cash Deposit is required. Charges will be made against this deposit if there is an emergency call-out, overtime inspection or when City
ordered barricading is required. Any such costs in excess of the deposit will be billed to the Permittee.
2. A one-year maintenance period and surety are required. Such period will begin on date of written acceptance by the City.
3. Refund or the cash deposit balance and refund or cancellation of the Faithful Performance Surety will be initiated by the written acceptance of the work
by the City.
4. The Permittee must request in writing a final inspection and acceptance of the work upon completion. Acceptance by the City will be made in writing
to the Permittee.
S. Maintain safe pedestrian and vehicular crossings and free access to private driveways, bus stops, fire hydrants, and water valves.
6. A Construction Traffic Control Plan and a Construction Schedule are required for all lane closures, detours, and street closures. This plan must be
reviewed and approved prior to any lane closures.
7. A Construction Traffic Control Plan shall conform to the Caltrans Manual of Traffic Controls for Construction and Maintenance Work Zones, dated
1990, available at Caltrans. Traffic control equipment shall include Type II flashing arrow signs if required.
8. Replace as directed by the City Engineer any damaged or removed improvements in accordance with City Standards and Specifications at the sole
expense of the Permittee.
9. Sawcut for all PCC or AC removals. All PCC removals shall be to the nearest scoremark and shall be doweled to existing improvements.
10. Prior approval of inspector is required for any work done after normal working hours, on weekends or holidays and may require reimbursement of
inspection costs at the current overtime rate.
11. Adequate signing and barricading is required on the job site. Failure to provide such signing and barricading may result in the City's providing signing
and barricades and charging the cost (including all labor and materials) against the cash deposit.
12. Compaction testing of subgrade, base rock, and asphalt concrete by Permittee is required unless otherwise stated by the City Engineer.
13. The Contractor or Permittee will have a supervisory representative available for contact on the project at all times during construction. Contractor or
Permittee shall provide a phone number at which they can be contacted outside the hours of 8:00 a.m. to 4:00 p.m.
14. No storage of materials or equipment will be allowed near the edge of pavement, the traveled way, or within the shoulderline which would create a
hazardous condition to the public.
15. This permit shall not be construed as authorization for excavation and grading on private property adjacent to the work or any other work for which a
separate permit may be required, nor does it relieve the Permittee of any obligation to obtain any other permit required bylaw.
16. This permit does not release the Permittee from any liabilities contained in other agreements or contracts with the City and any other public agency.
17. This permit is not transferable. Work must be performed by the Permittee or his designated agent or contractor as specified thereon.
18. Call back (call out) due to emeregencies regarding this permit shall be at the current overtime rate with a three (3) hour minimum charge per
occurrence.
19. Pursuant to Chapter 14.02 of the Campbell Municipal Code, applicant shall not cause to be discharged any material into the municipal storm drain
system other than storm water. Applicant shall adhere to the BEST MANAGEMENT PRACTICES established by the Santa Clara Valley Urban Runoff
Pollution Prevention Program.
20. If the public interest requires a modification of, or a departure from the plans and specifications, the City shall have the authority to require or approve
any modification or departure and to specify the manner in which the same is to be made for City-owned or maintained facilities.
21. Permitttee must provide advance notification to all parties that may be affected by the permit activities. Notification shall be reviewed by the City prior
to distribution and include dates of work and a contact name and phone number.
Applicant shall be responsible for ensuring that all those providing services under the applicant are aware and understand all of the above conditions.

ATSzT PROJECT LIGHTSPEED -SPECIAL PROVISIONS
1. AT&T shall provide the City of Campbell a current map indicating all SAI cabinet locations.
2. All proposed facilities shall be of consistent materials, finish and color (except where otherwise agreed, existing cabinets will be painted to
match paired Lightspeed cabinet). Pedestal numbering shall be consistent and as inconspicuous as possible. New Lightspeed cabinets shall have
an anti-graffiti finish.
3. Plans shall be accurately dimensioned, clearly showing the proposed location, size, and shape of all existing and proposed facilities, including
all existing and proposed right-of--way lines and any existing or proposed easements.
4. Applicant shall submit photos of the proposed location both pre-construction and post construction (with proposed structure(s) and all related
service pedestals, bollards, landscaping, screening, etc. shown).
5. Plans shall reflect that the following guidelines whenever possible. The proposed facilities should be:
a. located outside of any future sidewalk area
b. located outside and beyond the back of walk (existing or future) wherever possible, with a minimum clearance of 18" from the
sidewalk
c. in park strip locations (only considered if beyond the back of walk location is not feasible), located with a minimum clearance of
18" from the face of curb to any part of the facility (if bollards are proposed, bollards must be located a minimum of 18" from
face of curb), and a minimum of 18" from existing sidewalk
d. in sidewalk locations (only considered if beyond the back of walk locations are not feasible), located to accommodate the
required ADA specifications, with no less than a 4' clear path around the proposed facilities and with a minimum clearance of
18" from the face of curb to any part of the proposed facility (including bollards)
e. located 5' from any existing public improvements such as, trees (tree wells if applicable), light poles, sign poles, etc.
f. located with adequate clearance from existing fire hydrants
g. located with adequate line of sight ciearances
6. All contractors working under the permit shall have a valid City business license.
7. Applicant shall obtain approval from other agencies (Caltrans, Santa Clara Valley Water District, VTA, etc.) as needed.
8. After the City has reviewed and conditionally approved each permit location, the applicant shall proceed with the public/property owner
notification process as follows:
a. Following the City's preliminary approval of a site, AT&T shall mail a preliminary notice to all property owners that arc adjacent
to, or abut, the proposed location, as well as to those across the street from the location. In some cases, the City may request
notifications be sent to additional properties or parties. The preliminary notice, which shall be approved by the City prior to
mailing, shall be mailed to the property owners, with a copy to the City, not less than four (4) weeks prior to anticipated
construction date, and shall give the property owner a two (2) week period in which to respond with questions or concerns.
Should the notified property owner have concerns regarding the location of the facilities, AT&T shall work with the property
owner(s) toward a mutually acceptable location.
b. AT&T shall provide a final notice to the property owners at least 48 hours before the commencement of the proposed
construction.
9. Applicant shall ensure that all property owner notifications have been completed prior to construction.
10. AT&T agrees to maintain the facilities in an attractive and clean manner. AT&T will make commercially reasonable efforts to remove graffiti
within 48 hours of notification that the facilities have been tagged. AT&T shall provide the City with contact information for graffiti removal.
11. The City of Campbell will bill AT&T if its contractor leaves any site in such a manner requiring City forces to mobilize and mediate an unsafe
situation.
12. If and only to the extent required by State or federal law, should a public project arise that requires the relocation of existing facilities, the
facilities shall be removed/relocated to accommodate the scope, alignment and schedule of the public project, in accordance with state or federal
legal or regulatory requirements.
13. As required by State statues, and unless otherwise preempted by State or Federal Law, abandoned facilities shall be completely removed from
the site and the site restored to its original condition.
14. The parties agree to fully reserve their rights as to the applicability and interpretations of California Public Utilities Code Section 7901 and other
related local, state or federal laws, and the City reserves the right to contend that future permits will not be issued without AT&T obtaining a
franchise or other agreement prior to providing video services, and AT&T reserves it's right to contend that City is required to issue the
encroachment permits without first obtaining such franchise or agreement.

This is to notify you that AT&T California is upgrading the telecommunications network in
your neighborhood. The new network will be capable of delivering enhanced DSL, internet-
based telephone service, as well as new video services.
In order to bring this new technology to your neighborhood, it will be necessary to place an
above-ground cabinet. The cabinet is shown on the attached drawing and will be placed
according to applicable City guidelines. Although the cabinet will not be on your property,
we are sending this information to you as a courtesy. The cabinet will be installed in the
public right-of-way as indicated by the enclosed map.
After the upgrade of our network is complete, your neighborhood will have the most
advanced .and reliable communication services available. The City is currently considering a
decision on this network upgrade and we expect to be soon moving forward with
