1. Parld.ng stalls on west 1/2 of center parking area to be slanted in
opposite direction 80 th8t entrance can be limited to Campbell
Avenue and exit made to Poplar Avenue. Arrows and lettering on
pavement a. necessary to clarify pattern.
2. Owner to enter into written agreement with City Council to:
a. Install modified 6 foot cOIII88rcial sidewalk at this time.
Addlticmal 4 feet to be landacaped to conform with existing
development. 1ft in the future, the additional sidewalk is
needed, the City to give the Ainsley Corporation 30 days
notice to bring sidewalk up to ten (10') foot cClllDercia1
standard.
b. Replace dropped curb near westerly end of frontage on Campbell
Avenue with standard six inch curb.
c. Increase width of driveway on Campbell Avenue from 25 feet
to 30 feet.

In accordance with Section 9316, Chapter 3, Article IX of The
Campbell MUnicipal Code of the City of Campbell, the undersigned hereby
makes application for approval of the attached plans.
1. The said improvements will be located on land described as
follows: (Insert or attach legal description.)
Campbelll Medical Dental Center known as 621 East Campbell
Avenue. Property lies between the Easterly line of lots
3,4,5,6 Map of Pagels Subdivision of part of the formar Foote
Tract, in the city of CampbellWhich was filed in the office
of the county recorder, County of Santa Clara, State of Calif-
ornia April 15, 1907 im volume F2 of Maps page 41 and tha
Westerly line;, of lots: 9,and 10 Tract 122 Los Palos Unit #1
and all of lott 8 and a portion of lots 3, and 4 all in Tract
122 Los Palos Unit #1 Which Map was filed in the Recorder of
the CounJy of Santa Clara, State of California on March 18.
1940 in Book 4 of Mapa page 33.
2. The nature and pur'pos.:! of the proposed impt:ovements are:
To enable the completion of the Medical Dental Center
(Applicant should attach a "to.'ritten statement setting forth
any information he believes will be helpful to the Plan-
