Pursuant to the terms of Section 4IIiIIR of the Campbell Municipal Code, a permit to excavate within the public right of
way in the City of Campbell is issued based upon the following conditions:

The location of the job and the nature of the work to be done within the public right of way is as follows:

relating this to existing underground facilities.

No street shall be closed without full conformance to Section 7106 of the Campbell Municipal Code.
Safe crossings shall be maintained in conformance with Section 7105 of the Campbell Municipal Code.

Special Conditions:
Import backfill material as required by City Engineer.
Replace existing roadway structural section with equivalent type and thickness of material plus one additional
inch of asphalt; or 4 inches asphalt over 12 inches R-78 base rock, whichever is greater.
Work to be completed prior to occupancy of structure.
Work to be staked 3 feet back of face of curb by a licensed Land Surveyor or Civil Engineer and three copies of
the cut sheets sent to the Public Works Department before starting work.
Open cutting of pavement will NOT be allowed; all pipes or conduit shall be jacked under existing pavement.

Deposit (money or bond) required to guarantee restoration of public right of way.

The deposit in 6b above will be returned, in full, upon satisfactory restoration of public right of way.

The Permittee hereby agrees, by affixing his signature to this Permit, to hold the City of Campbell, its officers, agents, and
employees free, safe, and harmless from any claim or demand for damages resulting from the work covered by this Permit.

Acceptance of the Permit by the named Permittee constitutes acceptance of any and all conditions upon which this Permit
is granted. This Permit does not relieve the Permittee of any obligation to obtain any other Permit required by law.

1.
Property to be fenced and landscaped as indicated and as added in "red" on plans.
2.
Landscape plan indicating type of plant material, location of hose bibs or
sprinkler system and type of fencing to be submitted for approval of the
Planning Director at time of application for building permit.
3.
Landscaping shall be maintained in accordance with the approved landscaped plan.
4.
Faithful performance bond in the amount of $3,000 to be posted to insure land-
scaping, fencing and striping of parking area within three (3) months of
completion of construction, or applicant may file written agreement to complete
landscaping, fencing and striping of parking area prior to final Building
Department clearance.
5.
All mechanical equipment located on roofs shall be screened as approved by
the Planning Director.
6.
Trash enclosure to be at grade level.
The applicant is notified as part of this application that he is required to meet
the following conditions in accordance with Ordinances of the City of Campbell.
A.
All parking and driveway areas to be developed in compliance with Section 21.50
of the Campbell Municipal Code. All parking spaces to be provided with
appropriate concrete curbs or bumpel' qudl'ds.
B.
Undel'ground utilities to be provided as l'equÍl'ed by Section 20.16.070 of the
Campbell Municipal Code.
C.
Plans submitted to the Building Depadillent for plan check shall indicate clearly
the location of all connections fol' lmdel'g~'ollnd utilities including water, sewer,
electric, telephone and television cables, etc.
D.
Sign application to be submitted in dC,UI'ddllce viith pl'C)Visions of the Sign
Ordinance for all signs. No siqn to tw installed until lìpplicdtion is approved
and permit issued by the Building Depdrtlllent.
E.
Ordinance No. 782 of the Campbell ~1unicipdl Cocit' stipulates that any contract for
the collection and disposal of refuse, qal'baqe, wet gat'bage and rubbish produced
within the limits of the City of Call1pbt.' 1 1 "thill be IIlddL' with G¡'een Valley
Disposal Company. This requirement ap!Jlies to all sin~le family dwellings,
multiple apartment units, to all COllflle¡'cidl, business, industl'ial, manufactul'ing,
and construction establishments.
