BEING A RESOLUTION OF THE CITY COUNCIL
OF THE CITY OF CAMPBELL TO AUTHORIZE THE MAYOR TO
AMMEND AN AGREEMENT WITH SANTA CLARA COUNTY FOR
SENIOR NUTRITION SERVICES, FOR THE FISCAL YEAR 2004-05
WHEREAS, the City of Campbell recognizes the need for nutrition services for Senior Citizens
in Campbell; and
WHEREAS, the City agrees to share in the cost of operating the nutrition program, less
participant donations and transportation services; and
WHEREAS, the City and County of Santa Clara have entered into an agreement to provide
nutrition services for Senior Citizens in Campbell; and
WHEREAS, that agreement requires amendment due to an increase in availability of funds
from the County;
NOW, THEREFORE, BE IT RESOLVED that the Mayor is authorized to execute an
amendment to the agreement for Senior Nutrition Services with Santa Clara County.

Third Amendment to the Contract between the County of Santa Clara
and City of Campbell Contract Number SNPO3005
This is the third amendment to the contract between the County of Santa Clara (COUNTY)
and City of Campbell (CONTRACTOR) entered into on July 1, 2003 through for the
purpose of providing meals for senior citizens utilizing funds from the Older Americans Act of
1965, as amended, P.L. 89-73; 42 U.S.C Section 3000 et seq., and all amendments, rules
and regulations pertaining thereto.
Background
The purpose of this amendment is to allocate funds that were restored by the Santa Clara
Board of Supervisors to the FY 04-05 Senior Nutrition Budget. The additional funding is
included in the new Maximum Financial Obligation. The attached Meals and Program
Budget specifies the number of meals and fiscal budget for the extended term.
The contract is hereby amended as follows:
Amendment
1.
Article I, Section C, MAXIMUM FINANCIAL OBLIGATION will be deleted in its
entirety and replaced with the following: COUNTY will reimburse to CONTRACTOR
actual allowable expenditures subject to the provisions of this Contract, not to
exceed Forty-Two Thousand Nine Hundred Five Dollars ($42,905) in FY 03-04 and
Thirty-Six Thousand Three Hundred Seventy-One Dollars ($36,371) in FY 04-05.

Attachment C-05 Meals and Program Budget FY 04-05 will be deleted in its entirety
and replaced with the following: Attachment C-05 Meals and Program Budget FY
04-05 (Revision 1).
All other terms and conditions of the contract remain in full force and effect.

Please find enclosed an original amendment to the FY 04-05 Senior Nutrition Contract between your agency and
Santa Clara County to provide senior nutrition services.
The amendment allocates funds that were restored by the Board of Supervisors to the Senior Nutrition Program for
FY 04-05. A new budget is enclosed that supersedes any budget that your agency received previously.
Please review the enclosed budç¡et and note the chanç¡es. This budç¡et is retroactive to July 1. 2004 and
replaces the budç¡ets that were issued in July and or with the prior amendment.
Please have the appropriate authority sign, date and return to the Senior Nutrition Program. A copy of the
executed amendment will be returned for your records. Please return your completed amendment to the attention
