BEING A RESOLUTION OF THE CITY COUNCIL
OF THE CITY OF CAMPBELL TO AUTHORIZE THE MAYOR
TO EXECUTE AN AGREEMENT WITH OUTREACH FOR
CASE MANAGEMENT SERVICES, FOR THE FISCAL YEARS 2004-07
WHEREAS, the City Council of the City of Campbell recognizes the need for
case management services for the senior adult population of Campbell and;
WHEREAS, Outreach has been designated as the agency to provide case management
services and;
WHEREAS, funding has been approved in the Fiscal Year 2004-05 Operating Budget
for this purpose;
NOW, THEREFORE, BE IT RESOLVED that the Mayor is authorized to execute an
agreement for case management services with Outreach for Fiscal Years 2004-07, with
the option to renew through 2007-10 pending the availability of funds.
PASSED AND ADOPTED this 15th day of June 2004, by the following vote:
AYES: Council Members: Furtado, Watson, Kennedy, Burr
NOES: Council Members: None
