Dear Randy Westfall:
Enclosed please find a completed copy of the permit application you faxed to me to submit for a Blanket
Permit to place anchon; and ground rods in the City of Campbell. Once the Blanket Permit is approved and
a reference number shown on this permit we will begin the process.
Each installation will be identified On the location form by type of installation e.g. iUlChorl ground rod and
the surface type; dirt, concrete or asphalt and the address of each wOTl( operation. After the contractor calls
Underground Service Alert and the markings are complete. allowing the contractor to start installations,
they will fax to the City the locations to be worked by date. allowing the City a minimum of 24 hrs. to
prepare for field im;pection.
For installations in concrete the contractor will drill I core a pilot hole to allow me anchor and rod to pass
through for the installation; This procedure is predicated on the succe~.. of the final product and if approved
by the Public Works Inspector. Ground Rods placed within two ( 2 ) feet of the pole in concrete will
undergo the same scmtiny as the anchors.
Anchors I gt"ound rods are placed only when DCCCSsary and for the safety of the network.
A key map referencing streets within the node boundary will be sent as soon as it is highlighted and ready.
The contractor will be briefed on the City requirements and will be made to understand that complaints
should be handled immediately. (I recommend a prc-construction meeting prior to the commencement of
the start of installations), Please call if there are clarifications or ehanges required on 408. 491.8321
