Subject: Sidewalk Repair
Dear Ms. Tsai:
Hank West and I recently reviewed the condition of the sidewalk
fronting your property with Fred Faridnia of Pars Enterprise,
your on-site concrete contractor. We understand that you are
having him construct a new driveway and are interested in
having the sidewalk repaired at the same time.
The City currently has a sidewalk repair program wherein we
repair pedestrian trip hazards. Any improvements beyond that,
such as constructing a new driveway approach or improving the
appearance of existing concrete, remain the homeowner's
responsibility.
If you would like your contractor to repair the sidewalk, the
City would be willing to reimburse him for the amount of the
work that the City would have done under its sidewalk repair
program. The reimbursement would be based on unit prices that
the City currently pays its concrete contractor. The amount
of work and reimbursement total are shown below.

Any work you may choose to have done beyond that will be at
your expense, as well as any additional work needed to repair
damage to surrounding concrete done by your contractor during
construction.
CITY OF CAMPBELL
In order to have your contractor do the work, you need to obtain
an encroachment permit from the City's Public Works Department at
City Hall. An application is enclosed, as well as the name and
number of the inspector who would issue the permit. To help
facilitate the work, the City will waive the fees, deposits and
bond normally required of work in the public right of way.
If the work is confined to the repairs dicussed with your
contractor, then no drawing will be required. If additional work
is planned, a simple sketch will be required. The contractor must,
in any case, furnish a Workers' Compensation Certificate, fill out
the Information Sheet for Encroachment Permits, and furnish a
Certificate of Liability Insurance, naming the City of Campbell as
additional insured.
