SPECIAL PROVISIONS
PERMIT '91-141 HERITAGE CABLEVISION
1) Permittee shall pay the City for occupying shared trench the
amount of five (5) dollars per lineal foot of shared trench.
This payment shall be made prior to final release of this
permit. This provision is per agreement between the City of
Campbell Redevelopment Agency, Golden Bay Construction, and
Heritage Cablevision.
2) Permittee shall pay the City the amount of $137.00 for
additional digging required to place cable conduit in shared
trench. This payment shall be made prior to final release of
this permit. This provision is per agreement between the City
of Campbell Redevelopment Agency, Golden Bay Construction, and
Heritage Cablevision.
3) Permittee's work shall conform to the plans, specifications and
schedule of the Downtown Campbell streetscape project.
4) Where coordination is required between permittee and Golden Bay
Construction, permittee shall work at the convenience of Golden
Bay Construction.
5) Permittee shall supply to City an as-built showing depth, size,
and type of conduit, as well as all typical trench sections.
This as-built shall be approved by the city prior to final
release of this permit.
6) Where traffic control is required for permittee's work, a
traffic control plan shall be reviewed and approved by the City
prior to start of work requiring traffic control.

Heritage Cable Work in Downtown
Dear Mr. Wigney:
Thank you for your letter regarding the current work by Heritage Cable in
the Downtown Streetscape Project area. The work area of Heritage Cable
does overlap Golden Bay's work area.
However, we understand the issue to be resolved as follows: At your
request, a punch list for concrete and tree wells with grates was
developed after a walk-through on November 15, 1992. To the best of our
knowledge, Heritage Cable began work after the 15th, and is not involved
in any area where there is a punch list item. In the case of a conflict,
Heritage Cable's work is definable and will be recognized as the last work
in the area.

Dear Mr. Gomez,
Heritage Cablevision plans to occupy approximately 1135' of
the joint trench, install 1135' of conduit and 91 B-36 vaults (18"x
30"). We also plan to remove (2) existing vaults and install (2)
TV1024 pedestals on Central Ave. As previously indicated as to the
location of our conduit in the trench, there is no problem with our
duct being placed next to, on top or under the street light
conduit. To date we have placed approximately 410' of 2" conduit.

Dear Mr. Pitt:
The City just recently completed a very extensive and very costly project
of under grounding all of the public utilities facilities in this area of
Campbell Avenue from Winchester Boulevard to west of Railway Avenue and
from Civic Center Drive to Orchard City Drive. Your company participated
in both the planning and construction phases of that project. The City is
very concerned that Heritage Cablevision is proposing to install
additional new facilities within this area that could have been planned
and installed with the underground project.
Your referenced application proposes to install conduits, conductors and
service boxes that could have been installed at that time. The City is
very concerned with this piecemeal approach to providing service for
future customers in the downtown core.
The application that you have filed will be held in abeyance until you can
provide the City with a proposed master plan and schedule for providing
service to all areas within the underground district.
