Enclosed are the specifications for customer installed trenching, the trench inspection card
and a sketch showing the proposed trench route.
Upon completion of your trench work, please phone PG&E's Trench Inspector at (408)
725-2202 for inspection.
Installation of your service will be scheduled as soon as feasible after approval of your
work, provided all payments are received and permits are in order.
Please contact the telephone and CATV companies to advise them of your requirements
and request their recommendation for service.

GENERAL NOTES:
1. All the customer/contractor installed facilities and trenching must be inspected by a
PG&E Inspector. When approved, the Inspector will sign the Underground
Construction Inspection Card. Please make arrangements for inspections 48 hours
in advance by calling the number listed on the Inspection Card.
2. All materials and workmanship must meet the requirements and specifications as
outlined herein, and should not be Installed without prior notice to the Inspector. Any
material or workmanship not approved by the Inspector shall be corrected and
installed in an approved manner. No work shall be backfilled, covered, or in anyway
concealed until it has been approved by PG&E's Inspector.
3. The Customer is required to guarantee all material and workmanship for a period of
2 years after final acceptance by PG&E. Signature on the checklist constitutes the
Customer's recognition of the requirement.
