Estimate of work. The plans shall show the relation of the proposed work to existing surface and underground improvements. When approved by
the City Engineer, said plans become a part of this permit.
D. All work shall conform to the City of Campbell Standard Specifications and Details for Public Works Construction; the General Permit Conditions
listed on the reverse side; and the Special Provisions for this permit, listed below. Failure to abide by these conditions and provisions may result in
job shut-down and/or forfeiture of Faithful Performance Sureties and cash deposits. (See General Permit Conditions 1 and 2.)

Dear Mr. Lake:
As per our letter, dated April 11, 1997, regarding penalties to be assessed for the subject work; a
total of $200 will be added to the next quarterly billing as a revision to Permit #97-105. The
penalty was based on $100 per day for Monday, April 14th, and Tuesday, April 15th, during which
time the unsafe condition was not remedied. '
As we have discussed since that time, safety is a top concern to both PG&E and the City of
Campbell. Additionally, it is of the utmost importance that we maintain clear communication
and a good working relationship. It has come to my attention that there may be a few "built in"
problems in the timeframes written into the PG&E subcontractor agreements for trench
restoration work which do not comply with City requirements.
Let's set up a meeting as soon as possible to discuss our requirements, and how we can resolve

Dear Mr. Lake:
Pursuant to my phone message today, the City of Campbell is formally notifying
you of our intent to utilize section 11.34.010 of the Municipal Code providing the City
authority to assess a penalty of $1 00.00 per day for every day in which you fail to
complete the subject project. Moreover, the failure of PG&E to properly correct an
unsafe condition within the public right-of-way, warrants an additional $100.00 per day
penalty for every day the condition persists. The combined daily penalties to be assessed
total $200, and will become effective as of Monday the 14th of April 1997. As always,
Campbell Public Works Inspection personnel are available to answer any of your
questions regarding your responsibility in this matte~. Should PG&E be unable to
complete the above referenced work, please notify us so that we may schedule City forces
to perform the work on your behalf. Please note however, that the costs associated with
the use of City forces will be backcharged to PG&E.
