COMMENTS: It is understood that the building will not be expanded and that the parking lot
will not be changed.
Street Improvements: Prior to issuance of any grading, building permits, or use permit~
for the site, the applicant shall execute an agreement and shall cause plans for public
Street Improvements to be prepared, pay fees, post security and provide insurance
necessary to obtain an encroachment permit for construction of the public improvements,
as required by the City Engineer. The plans shall include the following:
. Removal of the existing two trees, the damaged curb and gutter, sidewalk, and
concrete in the parkway between the curb and sidewalk.
. Installation of new curb and gutter, sidewalk, street trees at 40 feet on center and
irrigation.
Utilities: All new on-site Utilities shall be installed underground per Section 20.36.150 of
the Campbell Municipal Code for any new or remodeled buildings or additions. Applicant
shall comply with all plan submittals, permitting, and fee requirements of the serving
utility companies.
Utility Installation Plan: Prior to issuance of building permits for the site, the applicant
shall submit a Utility Installation Coordination Plan and Schedule for approval by the City
Engineer for installation and/or abandonment of all utilities. Streets which have been
resurfaced within the previous 5 years will require boring and jacking for all new utility
installations. Applicant shall also prepare pavement restoration plans for approval by the
City Engineer prior to any utility installation or abandonment.
Maintenance Securitv: Prior to issuance of occupancy approval for the site, all public
street improvements required by the street improvement agreement and the encroachment
permit must be completed and accepted by the City and the applicant must provide a one-
year Maintenance Security in an amount of 25 % of the Faithful Performance Bond.

and shall comply with any requirements of the Santa Clara Valley Water District,
including Ordinance 83-2 and subsequent amendments.
Storm Water Pollution Prevention Measures: Prior to issuance of any grading or
building permits, the applicant shall comply with the National Pollution Discharge
Elimination System (NPDES) permitting requirements and the California Storm Water
Best Management Practices handbook prepared by the Storm Water Quality Task Force,
Santa Clara Valley Water District and the City of Campbell Municipal Code regarding
Storm Water Pollution Prevention.

I'm would like to apply for a use permit, to open up a Preschool Daycare Center located at
250 Virginia Ave. The ages of the children will be two and a half to five years old.
The amount of children to be cared for will be thirty-five with three preschool teachers
and two teachers aides and one Director. The hours of operation will be from 6:30 a.m.
to 6:00 p.m. There is also adequate parking in the church parking lot for the children to be
safely dropped off and picked up. I have enclosed the plans for the parking lot for the
planning department to review. The Church services are on Sunday are from 10:00 a.m.
to 12:00p.m. with a mid week service on Wednesday from 7:30 to 8:30 p.m.
'1 would like to open up a Preschool for the community at the Church located
at 250 Virginia Ave., understand there is a real need for quality daycare in this area.
I would like to apply for the Use Permit, please contact me as soon as possible.

COMMENTS: The exposed aggregate concrete around the large street tree is heaved upward
several inches. It must be removed and the parkway planted with grass. The sidewalk may
also be damaged. The tree may also have to be removed and replaced with an appropriate City
standard street tree and irrigation system. The situation will be evaluated prior to preparing
formal Conditions of Approval.

2) The proposed space must meet minimum light, ventilation and sanitation requirements
per UBC 308.5. Provide exit illumination per UBC 1012.
