RESOLUTION NO. 9888
A RESOLUTION EXTENDING THE CONTRACT
FOR CASE MANAGEMENT SERVICES WITH OUTREACH
WHEREAS, the City Council of the City of Campbell has authorized a contract
for the provision of case management services for the senior adult population of the City
of Campbell and;
WHEREAS, Outreach has been designated as the agency to provide case
management services and;
WHEREAS, funding has been approved in the Fiscal Year 2001/02 Operating
Budget for this purpose;
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of
Campbell that said Council extends the contract for doing the work, furnishing the
materials and supplies necessary to OUTREACH and that the Mayor of said City is
hereby authorized to extend the written agreement with the said contractor for Fiscal
Year 2001/02, and renewable through Fiscal Years 2002/03, 2003/04 pending the
availability of funds, and the City Clerk of said City is hereby directed to attest her
signature and affix thereto the appropriate seal of said City.
