RESOLUTION NO. 9760
A RESOLUTION AWARDING A CONTRACT
FOR CASE MANAGEMENT SERVICES
WHEREAS, the City Council of the City of Campbell has authorized a request
for proposals for the provision of case management services for the senior adult
population of the City of Campbell and;
WHEREAS, proposals were received and reviewed and an agency for these
services selected and;
WHEREAS, funding has been approved in the Fiscal Year 2000/01 Operating
Budget for this purpose;
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of
Campbell that said Council awards the contract for doing the work, furnishing the
materials and supplies necessary to OUTREACH and that the Mayor of said City is
hereby authorized to make and enter into a written agreement with the said contractor,
and the City Clerk of said City is hereby directed to attest her signature and affix thereto
the appropriate seal of said City.
PASSED AND ADOPTED this sixth day of November 2000, by the following
vote:
