STREET EXCAVATION PERMIT NO.
Pursuant to the terms of Section 7101 of the Campbell Municipal Code, a permit to excavate within the public right of
way in the City of Campbell is issued based upon the following conditions:

~ Import backfill material as r~uired by City Engineer.
~ Replace exi~ing roadway structural section with equivalent type and thickness of material plus one additional
inch of asphalt; or 4 inch~ asphalt over 12 inches R-78 base rock, which~er is greater.
~ Work to be completed prior to occupancy of structure.
~ Work to be s~k~ 3 feet back of face of curb by a licensed Land Surveyor or Civil Engineer and three copi~ of
the cut sheets sent to the Public Works Department before ~arting work.
~ Open cutting of pavement will NOT be allowed; all pipes or conduit shall be jacked under existing pavement.

The Permittee hereby agrees, by affixing his signature to this Permit, to hold the City of Campbell, its officers, agents, and
employees free, safe, and harmless from any claim or demand for damages resulting from the work covered by this Permit.
Acceptance of the Permit by the named Permittee constitutes acceptance of any and all conditions upon which this Permit
is granted. This Permit does not relieve the Permittee of any obligation to obtain any other Permit required by law.

BEING'A RESOLUTION OF THE CITY .COUNCIL OF TIlE CITY
OF CAMPBELL APPROVING A USE PERMIT, INCLUDING PLANS
AND ELEVATI6NS FOR A PORTION OF TIlE INTERIM ZONE
AS ESTABLISHED BY ORDINANCE 916 ON THE 24TH DAY OF
SEPTEMBER, 1973, AND EXTENDED BY ORDINANCE 930 ADOPTED
ON THE 14TH DAY OF JANUARY, 1974, AND ORDINANCE 956-A
ADOPTED ON SEPTEMBER 10, 1974.
(Application of Bruce G. Johnson).
The City Council of the Ci.ty of Campbell does hereby ordain as follows:
SECTION ONE: That a use permit, including plans and elevations, is
hereby approved on that portion of an Interim Zone known as 80 Alice Avenue
as per the application of Mr. Bruce G. Johnson.
Said approval granted subject to the following conditions:
Underground utilities to be provied as required by
Secti'On 20.16.070 of the Campbell Municipal Code. All
parking spaces to be provided with appropriate concrete
curbs or bumper guards.
Plans submitted to the building department for plan check
shall indicate clearly the location of all connections for
underground utilities, including water, sewer, e!ect'ric,
telephone and television cables, etc.
Ordinance No. 782 of the Campbell Municipal Code stipulates
that any contract for the collection and di.sposal of refuse,
garbage, wet_ garbage and rubbish produced wi thin the limits
of the City of. Campbell st]all be made with Green Valley
Disposal Company. This requirement applies to all single
family dwellings, multiple apartrnent units, to all commercial,
business, industrial, manufacturing, and construction estab-
I ishments.
BUILDING DEPARTMENT.
Comply with Chapter 37 of the Uniform Building Code for fireplace
requirements.
