RESOLUTION TO AMENDMENT TO THE CITY OF CAMPBELL
CONTRACT WITH EMPLOYEE BENEFIT SPECIALISTS (EBS), SECTION 125
FLEXIBLE BENEFITS CAFETERIA PLAN
WHEREAS, City of Campbell (the "Agency") has previously adopted the City of
Campbell EBS Section 125 Flexible Benefits Plan (the "Plan"); and
WHEREAS, the Agency has the right to amend that Plan; and
WHEREAS, the Agency deems it to be in the best interest of the Agency and the Plan
to amend the Plan.
NOW, THEREFORE, BE IT RESOLVED, the plan is hereby amended as follows,
effective January 1, 2012:
Claims
1. Claims for reimbursement for an active Participant must be filed with the Plan
Administrator within 90 days following the last day of each Plan Year.
2. The Plan does not provide for an earlier deadline for claims submission for
Terminated Participants.
3. Claims for reimbursement for a Terminated Participant must be filed with the plan
Administrator within 90 days following the last day of each Plan Year.
